TERMS AND CONDITIONS
1. (I/We) understand that (my/our) child(ren) will be unable to attend Camp Gan Izzy unless and until the state required good health and immunization certification is received from a physician.

2. (I/We) understand that (my/our) child(ren) are enrolled for camp sessions indicated. The camp cannot issue refunds or credits for illness, holidays or family vacations.

3. In the event that the camp is closed due to or resulting from a weather emergency or other unforeseen circumstance, there will be no make-up days, refunds or credits for days that school is not in session.

Payment Information:
Payments may be made by ACH or credit card.
(Deposit must be made by ACH, credit card, or Zelle)
All credit card charges will be assessed a 3% non-cash adjustment fee by the processor.
Monthly payments are due February 15, 2025 - May 15, 2025

Withdrawal:
Prior to February 1, any payments will be refunded except for the $150 deposit.

After submitting this registration form, you will receive an email with the PAYMENT Link for the registration fee and deposit. Click the MAKE PAYMENT button and pay $150 per child, non refundable by either ACH or credit card. Your child(ren) are not registered for camp until this payment is made. All credit card charges will be assessed a 3% non-cash adjustment fee by the processor. ACH/Debit Card payments do not incur a fee.
